Use the DS-82 form if your passport has a 10-year validity & you use the same name on your recent passport, or your name was changed by marriage, divorce, or court order longer than One (1) year ago. If your name changed less than one year ago you will require form (DS-5504).

1.    Renewal FORM DS-82 – (on-line form filler found on www.travel.state.gov) Fill out on-line form then print and sign in black ink only. No blank answers please. (call for assistance)

2. ONE (1) PASSPORT PHOTO – in color, size 2 by 2. They must have a WHITE BACKGROUND.

No eyeglasses or head coverings unless for religious purposes or medical reasons. If so, an accompanying letter explaining why is required from applicant.

3. CURRENT U.S. PASSPORT – Your passport must have been issued within the past (15) years and not damaged. * Please check your passport. If your passport was a 5-year validity or has been issued over 15 years ago you must apply for a New Adult Passport.  If your old passport is damaged, please call our office (713) 870-0078 before submitting your application.

4. PROOF OF DEPARTURE – If tourism then an Original Ticket, E-Ticket, Cruise Confirmation or Hotel Reservation must be presented if passport is required within two (2) weeks.

    If for business travel then a company letter is allowed. (call for example letter)   

5. ONE (1) LETTER OF AUTHORIZATION – (Select Reference Menu) Please make sure the first 2 middle boxes that state  “I authorize” are checked. Below that, please print the travelers name, phone number and TODAY’S date. Sign the bottom line like your signature in your current passport.

6. STATE FEES – A check or money order in the amount of $208.32 MADE PAYABLE TO “U.S  Department of State” . (We can write the check) 

7. OUR ORDERFORM – (Select Reference Menu) Your personal information such as name, address and phone and your payment method.

8. ADDITIONAL DOCUMENTS – If name is changing due to Marriage, Divorce or Court Order then please provide original documents whether a marriage license, divorce decree or official court documents showing name change. Make copies of these documents for your records.

   All original documents are returned by the agency with the new passport.
